Please fill out the following form in its entirety to reserve a plaque on the memorial of the new Ocean Beach Veterans Plaza. All veterans will be verified by the U.S. Department of Veterans Affairs for accuracy/ service dates.
The memorial plaques will have a uniform look designed to symbolize the camaraderie of service. The logo for each veteran’s branch of service will be at the top of each plaque. Inscriptions will be limited to a total of three lines:
First Line: Veteran’s Rank and Name
Second Line: Branch of Service and Dates of Service
Third Line: Medals and Citations for Valor in Combat
Priority will be given to all names from the original Veterans Plaza.
A onetime donation of $300 will be charged upon application submittal to be used to purchase the plaque and for plaza upkeep. Refund of the donation will be processed if the veteran’s name is not included in the project.
Sponsorships are available for veterans and families that would like a plaque but are unable to donate. Please contact the Ocean Beach Community Development Corporation for more information.
